چکیده

BACKGROUND Proximal humerus fractures are one of the most common fractures among elderly patients. We performed a systematic review to detail the demographics, outcomes, and complications of patients who undergo reverse shoulder arthroplasty for complex proximal humerus fractures. METHODS A systematic review of the literature was performed. Two reviewers assessed and confirmed the methodical quality of each study. Studies that met our criteria were assessed for pertinent data, and when available, similar outcomes were combined to generate frequency-weighted means. RESULTS Nine studies met the inclusion and exclusion criteria for this review. The frequency weighted mean age was 77.5 years and the mean follow up was 43.2 months. Females comprised 90.4% of the patient population. Four-part fractures were most commonly encountered. The frequency weighted mean constant's score was 55.9. Frequency weighted mean active forward flexion, abduction, and external rotation at neutral were 122°, 97°, and 18°, respectively. Tuberosity repair was associated with significantly higher external rotation compared to no repair (24° vs 15°; P = .0003). The most common complication was scapular notching (32%) while the impact of this finding remains unknown. CONCLUSION Pooled data and frequency weighted mean outcomes showed that RSA patients tend to be elderly women and still have postoperative dysfunction despite well-controlled pain. Repair of the greater tuberosity allows greater range of motion in patients.

Early Versus Late Reverse Shoulder Arthroplasty for Proximal Humerus Fractures: Does It Matter?

Background: This study compared the outcomes between patients with proximal humerus fractures (PHF) whounderwent acute reverse total shoulder arthroplasty (RSA) to those who underwent an alternative initial treatmentbefore requiring (secondary) RSA.Methods: Patients who underwent RSA after suffering a PHF were identified. Two year clinical follow-up was requiredfor inclusion. Patients were divi...

Comparison of Reverse Total Shoulder Arthroplasty vs Hemiarthroplasty for Acute Fractures of the Proximal Humerus: Systematic Review

The optimal treatment of proximal humerus fractures is debated; however, reverse total shoulder arthroplasty (RTSA) has become increasingly popular as the primary treatment. We systematically reviewed the PubMed, EMBASE, and Scopus databases to identify English-language clinical studies (evidence levels I through IV) comparing the results of RTSA with those of hemiarthroplasty (HA) for the trea...

Hemiarthroplasty versus reverse total shoulder arthroplasty for acute proximal humerus fractures in elderly patients.

Proximal humerus fractures are the third most common fracture in elderly patients. Hemiarthroplasty has been the treatment of choice in patients with bone quality and fracture patterns not amenable to open reduction and internal fixation. Reverse total shoulder arthroplasty is a newer option that appears to be less dependent on tuberosity healing than hemiarthroplasty. The authors hypothesized ...

The Role of Reverse Shoulder Arthroplasty in Management of Proximal Humerus Fractures with Fracture Sequelae: A Systematic Review of the Literature

Fracture sequelae of the proximal humerus poses a complex management decision due to the frequent deformity and its consequences on the peri-articular soft tissues. These patients are frequently elderly with significant medical comorbidities. Due to the age of the patient there is frequently rotator cuff deficiency and therefore the reverse shoulder arthroplasty (RSA) becomes the arthroplasty o...

Functional outcomes of reverse shoulder arthroplasty compared with hemiarthroplasty for acute proximal humeral fractures.

BACKGROUND Complex acute proximal humeral fractures may require prosthetic replacement of the proximal humerus. Reverse shoulder arthroplasty (RSA) has been suggested as an alternative to hemiarthroplasty in the management of such fractures. This study compared the functional outcomes of RSA with hemiarthroplasty in patients with acute proximal humeral fractures. MATERIALS AND METHODS All pat...
